Job summary

Bay Cove Human Services, based in Greater Boston and Southeast Massachusetts, seeks a Case Manager to deliver intensive, culturally sensitive, person-centered housing services using motivational interviewing, trauma-informed care, and harm reduction principles.

The role entails assessing housing barriers, developing ISP plans at least every 90 days, assisting with housing searches, move-in supports, and ensuring connection to benefits and services to promote housing stability.

Qualifications

• Bachelor Degree required.
• At least two years case management and/or direct care experience in areas such as homeless services/housing search, substance use treatment, mental health, and/or harm reduction services.
• Familiarity with affordable housing and housing subsidies preferred.

Responsibilities

• Assess participant concerns and barriers to housing at intake and annually thereafter.
• Create person-centered, strengths-based service plans (ISP) reviewed at least every 90 days.
• Assist with housing search, including contacting landlords and accompanying tenants to view/apply for units.
• Provide move-in transition supports, budgeting assistance, and access to resources.
• Advocate for benefits such as SSI/SSDI, SNAP, unemployment, health insurance, and fuel assistance.
• Develop crisis intervention plans and provide strengths-based interventions during psychiatric distress.
• Conduct home visits and maintain relationships to promote housing stability and community supports.
• Educate and coach activities of living skills (ALS) and assist with banking, shopping, and daily tasks.
• Complete data entry and record keeping for ISPs, consents, incident reports, case notes, and referrals.
• Build partnerships with internal and external service providers to ensure comprehensive services.
