Job summary

HomeFirst seeks a dedicated Case Manager for the Emergency Interim Housing (EIH) program at the Cherry site. You will manage a caseload, develop individual plans, and coordinate with community partners to support residents toward stable housing.

Responsibilities include intake, documentation, and trauma-informed, client-centered care in a 24/7 staffed facility. Strong organizational and collaborative skills are essential, with onsite work preferred.

Qualifications

• Bachelor’s degree (or 2 years case management experience)
• Experience building relationships with community partners
• Ability to manage multiple projects with deadlines
• Strong written and oral communication skills
• Ability to work with diverse socio-economic backgrounds
• Preference for extensive onsite work

Responsibilities

• Welcome guests and participants to the facility
• Maintain a caseload with case management and direct services
• Develop and implement individual case management plans
• Provide community-based case management and referrals
• Collaborate with community partners and EIH team
• Coordinate with external providers to reach housing goals
• Conduct intake and program documentation
• Maintain records per grant and compliance standards
• Participate in Case Conferencing to support housing goals
• Provide trauma-informed care and CTI-based services
