HomeFirst Home Healthcare is seeking a Hybrid Case Manager in San Jose to join an EIH Via Del Oro program. You will work with adults to develop housing plans, conduct intake, and coordinate services to move participants toward permanent housing.

The role requires strong case management skills, collaboration with community partners, and adherence to program policies. The position offers a hybrid schedule, non-exempt full-time hours, and a team-focused environment dedicated to ending homelessness

Qualifications

• Minimum two years’ experience in social services.
• Lived experience or understanding of the unhoused population.
• Bi-lingual capabilities a plus.
• Fluent in Microsoft Office Suite.

Responsabilités

• Welcome agency guests into the facility and assist participants with intake.
• Maintain a caseload and provide case management services.
• Develop individualized housing plans and support participants to achieve housing goals.
• Collaborate with partners to coordinate services and avoid duplication of efforts.
• Complete safety planning, risk assessment and mandated reporting.
• Attend meetings and participate in program conferences and staff development.
