Job summary

HomeFirst seeks a Case Manager for the Emergency Interim Housing Via Del Oro program in San Jose. The role focuses on individualized housing plans, direct case management, and coordination with partners to move participants toward permanent housing.

The position is full-time, non-exempt, reporting to the on-site Program Manager. Candidates should have at least two years in social services or a relevant bachelor’s degree, with strong communication and problem-solving skills.

Qualifications

• Minimum two years’ experience in social services.
• Lived experience OR understanding of and sensitivity to the unhoused population.
• Bi-lingual capabilities a plus.
• Fluent in Microsoft Office Suite.

Responsibilities

• Welcome agency guests into the facility including program participants, volunteers and staff.
• Maintain a caseload and provide case management and other direct services to all participants.
• Develop individualized housing plans with participants and provide support to help achieve goals.
• Collaborate with community partners to coordinate support services for participants.
• Complete safety planning, risk assessment and mandated reporting.
• Maintain thorough, accurate records of case management activities in HMIS.
