Job summary

Church at the Park is seeking a Case Manager to support individuals experiencing homelessness in a shelter setting, guiding their transition to stable housing. You will coordinate with community partners to enroll in housing programs, obtain documentation, and connect to local resources, including OHP.

You may focus on youth, singles, couples, or families, and will participate in outreach and community meetings to strengthen coordinated responses. A valid Oregon driver's license is required.

Qualifications

• Two years of case management or social services experience.
• Strong collaborative skills with case managers, healthcare providers, and social service agencies.
• Experience with homelessness, chronic illness, behavioral health challenges, or substance use disorders.
• Strong organizational skills and ability to manage multiple participants' needs.
• Must have valid Oregon driver's license and driving record eligibility.
• Pass preemployment drug screening.

Responsibilities

• Maintains thorough, accurate, and timely documentation of participant interactions, services provided, and progress toward goals.
• Submits required data, case notes, and reports per program and grant requirements.
• Ensures all protected health information (PHI) is handled in compliance with HIPAA and confidentiality standards.
• Participates in training and professional development initiatives to strengthen team capacity.
• Coordinates medication pickup and delivery for participants as needed, ensuring secure handling and completion of receipts.
• Coordinates participation in outreach and community meetings to support coordinated responses and resource connections.
