Job summary

The People Concern in Los Angeles seeks a dedicated Case Manager to support clients who are homeless through harm-reduction, housing-first philosophy, and trauma-informed care. You will assess barriers to housing, coordinate services, and work toward successful housing placement and retention for each client.

You will maintain client records, collaborate with local agencies, and provide life skills coaching to promote independent living.

Qualifications

• High School Diploma or GED required.
• Bachelor's degree preferred; or minimum two years’ experience working with homeless individuals.
• Valid CA Driver’s license, reliable car, auto insurance, and acceptable driving record.
• Willing and able to transport Clients in your personal vehicle.
• Able to work in a high tolerance Harm Reduction model with Clients experiencing multiple barriers.

Responsibilities

• Maintain a caseload of approximately 20 high acuity Clients for intensive case management services.
• Develop relationships with Clients with a focus on facilitating independence and maintenance of improved health.
• Coordinate with LA County DMH and DHS programs to enhance coordination within Clients’ treatment teams.
• Engage Clients with harm-reduction and trauma-informed care while upholding professional boundaries.
• Assist in screening, intake, and enrollment of Clients and orientation to program policies and goals.
• Complete biopsychosocial assessments and functional needs assessments to guide treatment planning.
• Maintain progress notes and data in electronic databases as required by funding sources.
• Ensure all Client documents are complete, accurate, and filed according to guidelines.
