Job summary

Church at the Park is seeking a Case Manager to support individuals experiencing homelessness in a shelter setting, guiding their transition to stable housing. You will coordinate with community partners to enroll in housing programs, obtain documentation, and connect to local resources, including OHP.

You may focus on youth, singles, couples, or families, and will participate in outreach and community meetings to strengthen coordinated responses. A valid Oregon driver's license is required.

Qualifications

  • Two years of case management or social services experience.
  • Strong collaborative skills with case managers, healthcare providers, and social service agencies.
  • Experience with homelessness, chronic illness, behavioral health challenges, or substance use disorders.
  • Strong organizational skills and ability to manage multiple participants' needs.
  • Must have valid Oregon driver's license and driving record eligibility.
  • Pass preemployment drug screening.

Responsibilities

  • Maintains thorough, accurate, and timely documentation of participant interactions, services provided, and progress toward goals.
  • Submits required data, case notes, and reports per program and grant requirements.
  • Ensures all protected health information (PHI) is handled in compliance with HIPAA and confidentiality standards.
  • Participates in training and professional development initiatives to strengthen team capacity.
  • Coordinates medication pickup and delivery for participants as needed, ensuring secure handling and completion of receipts.
  • Coordinates participation in outreach and community meetings to support coordinated responses and resource connections.

Position Summary

This position works with individuals experiencing homelessness in a shelter setting andis responsible forsupporting their transition from shelter to stable, permanent housing. The Case Manager collaborates closely with community partners to coordinate services andparticipatesin joint efforts to help participants complete housing assessments, enroll in the Oregon Health Plan (OHP), obtain documentationrequiredfor housing and benefits, and connect to local resources. The role also includes attending community meetings, contributing to coordinated community responses, andparticipatingin outreach efforts as needed. The Case Manager may focus on one or more service areas, including case management for young adults ages 18-24, case management for singles, couples, and families, and/or community health work that integrates health and housingsupports.

Preferred Qualifications
  • Associate degree in human services, social work,orsociology preferred.
  • Two (2) years of case management or social services experience.
  • Demonstrated experience in mediation between personsexhibitingescalated behavior.
  • Strong ability to work collaboratively with other case managers, healthcare providers, and social service agencies.
  • Experience working with individuals experiencing homelessness, chronic illness, behavioral health challenges,and/or substance use disorderspreferred.
  • Strong organizational skills and ability to manage multiple participants' needs effectively.
Other Requirements
  • MUST have a valid Oregon Driver’s license; must have been a licensed driver for at leastthree (3)years; must have a driving record thatpermitscoverage under C@P’s company insurance.
  • All applicants must successfully pass a preemployment drug screening prior to beginning employment.
