Qualifications

Responsibilities

• Manage a caseload of up to 20 households in transition to stable housing.
• Attend encampment and housing placement coordination with outreach teams.
• Assist with housing applications, vouchers, and move-ins within timeframes.
• Document client interactions and outcomes in HMIS.
• Provide one year of weekly, in-person case management in clients' homes.
• Monitor progress and adjust service plans to ensure housing stability.
• Support transportation and connections to behavioral health and healthcare services.
