Job summary

Community Housing Innovations (CHI) in Medford, NY is seeking a dedicated full-time Case Manager to support individuals in achieving stable housing and economic independence. You will conduct face-to-face meetings, assess needs, develop Independent Living Plans (ILP), provide case management, referrals, and advocacy while ensuring program compliance and timely reporting.

Candidates should have a Bachelor’s degree in human services or a related field; a Master’s in Social Work is preferred.

Qualifications

• Bachelor’s degree in human services or related field required.
• Master’s degree in Social Work preferred; extensive experience may substitute.
• Two years’ experience in homelessness or housing preferred.
• Proficiency in English; bilingual Spanish a plus.
• Strong communication, budgeting, and organizational skills.

Responsibilities

• Conduct face-to-face meetings with clients.
• Assist in developing Independent Living Plans (ILPs) and track progress.
• Provide information, referrals, counseling, crisis intervention, and advocacy.
• Develop a resource network and collaborate with DSS and service providers.
• Assist clients with budgeting, resumes, ADL skills, and related tasks.
• Complete discharge plans and timely internal/external reports.
