Job summary

Community Housing Innovations, Inc. (CHI) is seeking a full-time Case Manager in our Wading River, NY location to support participants in achieving housing stability and independence.

You will coordinate services, assist with budgets and ILPs, and collaborate with partner agencies to ensure high-quality outcomes. Strong communication, bilingual skills a plus, and ability to work independently and with a team are essential.

Qualifications

• Bachelor’s degree in human services or related field required; extensive experience may substitute.
• Master’s in Social Work preferred but not mandatory for all applicants.

Responsibilities

• Ensure clients receive services to promote independence and economic self-sufficiency.
• Develop Independent Living Plans (ILP) and monitor progress toward goals.
• Provide information, referrals, counseling, crisis intervention, direct service, and advocacy to clients.
• Develop and maintain a network of referrals; collaborate with DSS and other service providers.
• Assist clients with budgets, resumes, ADL skills, and face-to-face meetings; inspect housing units as needed.
• Submit reports timely; maintain charts and files; ensure program compliance.
