Job summary

Institute for Community Living Inc. (ICL) is seeking a dedicated Case Manager to support residents in achieving independence and stable housing through comprehensive case management, service planning, and coordination of care.

The role emphasizes developing individualized service plans, maintaining documentation, and collaborating with internal staff and external providers to ensure access to housing, healthcare, and support services. CPR/First Aid certification is required.

Qualifications

• Bachelor's degree in health or human services field.
• Three years of case management or related human services experience.
• CPR and First Aid certifications (must obtain and maintain).

Responsibilities

• Participate in orientation of new residents by explaining available services.
• Obtain and document assessment information.
• Collaborate with residents to develop and maintain service plans.
• Provide supportive counseling and skill-building.
• Refer and accompany residents to community resources.
• Assist with budgeting and independent living skills.
• Advocate on behalf of residents.
• Maintain accurate case records.
