Job summary

ICL seeks a dedicated Senior Case Manager to support consumers living in independent community apartments. You will help develop and maintain living, working, and social plans, coordinating medical, psychosocial, educational, and vocational services to promote independence.

Qualifications

• Bachelor's degree in human services or related field plus relevant experience in direct services.
• Experience providing services to individuals with mental disabilities is preferred.
• Strong communication and documentation skills required.

Responsibilities

• Provide direct supportive counseling in 1:1 or small group settings.
• Develop and maintain consumer treatment plans and progress notes.
• Coordinate access to medical, psychosocial, educational, and vocational services.
