Job summary

Institute for Community Living Inc. (ICL) in New York City seeks a Case Manager for Supported Housing. You will join a team delivering community-based services, coordinating access to medical, psychosocial, financial, vocational, and daily living supports to individuals living in independent apartments.

Under supervision, you will conduct visits, maintain case records, provide crisis intervention, and advocate for clients to access training and employment opportunities.

Qualifications

• Bachelor’s degree plus two years’ direct service experience.
• Alternatively, bachelor’s with required practicum and one year experience.
• Or an associate’s degree plus three years of relevant experience.

Responsibilities

• Travel to/visit consumers’ residences to provide supportive services on a schedule.
• Counsel consumers individually or in groups to meet service needs and goals.
• Maintain accurate, up-to-date case records and measurable service plans in compliance with standards.
• Provide crisis intervention and assist with first aid/CPR when needed.
• Assist consumers in accessing vocational training and education opportunities.
• Advocate on behalf of consumers and coordinate community supports.
