Job summary
Institute for Community Living Inc. (ICL) is seeking a dedicated Case Manager to support residents in obtaining stable housing and coordinating essential services. You will develop individualized plans, document progress, and collaborate with staff and external providers to ensure comprehensive care.
The role emphasizes advocacy, documentation, and goal attainment, with onsite work and a strong commitment to community welfare.
Qualifications
- Bachelor's degree in health or human services fields such as social work, psychology, nursing, rehabilitation, education, occupational therapy, recreation, counseling, or sociology.
- Three years of case management experience or other relevant human services work is required.
- CPR and First Aid certifications must be obtained and maintained.
Responsibilities
- Assist residents in achieving independence and stable housing through case management and service planning.
- Develop individualized service plans and maintain documentation (ILPs, psychosocials, progress notes).
- Coordinate with internal staff and external providers to ensure housing, healthcare and support services.
- Advocate for residents and help them set and meet personal goals.
