Job summary

ICL is seeking a Case Manager to support residents in achieving independence and stable housing. You will develop individualized service plans, coordinate care, and document progress to ensure housing, healthcare, and support services meet residents’ needs.

The role emphasizes advocacy, collaboration with staff and external providers, and maintaining detailed records to track outcomes. Onsite work is required in New York.

Qualifications

• Bachelor’s degree in health or human services field.
• Three years of case management or related human services experience required.
• CPR and First Aid certifications (must obtain and maintain).

Responsibilities

• Orient new residents by explaining available services.
• Document and document assessment information.
• Collaborate with residents to develop and maintain service plans.
• Provide supportive counseling and skill-building.
• Refer residents to community resources and accompany them to services.
• Assist with budgeting and independent living skills.
• Advocate on behalf of residents.
• Maintain accurate case records.
• Attend meetings and trainings.
• Report incidents and comply with policies.
• Ensure CPR and First Aid certifications are up to date.
