Institute for Community Living Inc. (ICL) is seeking a Case Manager for its Supported Housing program.

The role supports individuals living in independent apartments, coordinating access to medical, psychosocial, financial, and vocational services to promote stable living and community integration. The Case Manager delivers crisis intervention when needed, accompanies clients to appointments, and participates in supervision and trainings to ensure quality care and adherence to regulatory

Qualifications

• Education: Bachelor's degree with 2 years direct service experience or equivalent.
• Direct service experience with mental disabilities or relevant human services.

Responsibilities

• Visit consumers at residences and provide supportive services on schedule.
• Assist in improving attitudes and behavior through supported counseling.
• Coordinate access to medical, psychosocial, financial, and vocational services.
• Maintain case records, assessments, and service plans with quarterly reviews.
• Deliver crisis intervention and connect to community supports.
