Job summary

ICL is seeking a Case Manager to support individuals living in independent community apartments. You will coordinate access to medical, psychosocial, financial, vocational, and other essential services, delivering crisis intervention as needed to maximize independence.

The role requires field-based service delivery with regular travel to client residences and strong collaboration with the Program Director and team.

Qualifications

• Bachelor's degree with 2 years’ direct service experience or bachelor’s degree with a required practicum.
• Experience providing direct services to individuals with mental disabilities or related human services.
• Work eligibility to work in the U.S.

Responsibilities

• Travel to consumers’ residences to provide supportive services.
• Assist consumers in modifying attitudes and behavior.
• Complete assessments, service plans, and treatment records.
