Job summary

The People Concern is seeking a Case Manager to work onsite at Redtail Crossing Apartments in Los Angeles, supporting clients from homelessness to permanent housing. You will conduct assessments, coordinate services, and maintain case notes to promote housing stability and self-sufficiency.

This role requires strong collaboration with county agencies, trauma-informed care, and a commitment to client-centered, harm-reduction approaches within a 100% onsite setting.

Qualifications

• High School Diploma or GED or Equivalent.
• Experience with homeless individuals or intensive case management.
• Knowledge of issues faced by a culturally diverse population.

Responsibilities

• Maintain a caseload of high acuity Clients for intensive case management.
• Coordinate housing placement and retention support with clients.
• Collaborate with LA County DMH and DHS programs for treatment team coordination.
• Document progress notes and maintain confidential client information.
