Job summary

The Coalition for Responsible Community Development in Los Angeles is seeking an ICMS Case Manager. This full-time role involves providing coordinated case management for formerly homeless individuals diagnosed with special needs. Responsibilities include conducting housing assessments, developing Individual Service Plans, and assisting with access to various services.

The ideal candidate will have a BA degree, experience with youth facing housing instability, and strong communication and organizational skills. Competitive compensation and benefits are offered.

Qualifications

• 1-2 years of experience or related work.
• Experience in foster care, probation, or mental health systems.
• Reliable, flexible team player.

Responsibilities

• Conduct initial housing intake and needs assessment.
• Assist tenants in determining personal goals through Individual Service Plan.
• Conduct face-to-face contacts and progress meetings with tenants.
• Act as liaison among various stakeholders to communicate concerns.
• Provide information regarding tenant rights and housing rules.

Skills

Experience with youth experiencing homelessness

Proficient in Microsoft Office

Good work ethic

Reliable transportation

Ability to maintain personal boundaries
