Job summary

The People Concern is hiring a Temporary Case Manager for Project Based Housing in Los Angeles. This onsite role supports housing stability using harm reduction, housing first, and trauma‑informed care to help clients achieve independence and community integration.

You will collaborate with DMH and DHS teams, coordinate services, and guide clients toward sustainable living arrangements, with a focus on skill development and equitable access to resources.

Qualifications

• Valid CA driver’s license and reliable vehicle required.
• Must be able to transport Clients in personal vehicle.
• Experience serving individuals who have homeless history or dual-diagnosis.
• Cultural competence in a diverse population.
• Proficiency with Word, Excel, and Outlook.

Responsibilities

• Maintain a caseload of ~20 high acuity Clients for intensive case management.
• Develop relationships to promote independence and health maintenance.
• Collaborate with DMH and DHS programs for coordinated care.
• Engage Clients with harm‑reduction and housing‑first approaches.
• Assist with intake, orientation, and enrollment; document goals.
• Conduct biopsychosocial assessments to guide treatment planning.
• Write quarterly goals to improve quality of life and health outcomes.
• Maintain accurate progress notes in electronic databases.
• Ensure client documents are complete and properly filed.
• Maintain HIPAA confidentiality and agency guidelines.
• Help Clients apply for and obtain entitled benefits.
• Coordinate appointments, transportation, and follow-up services.
• Transport Clients to appointments across LA area as needed.
• Teach life skills (budgeting, housing, health maintenance).
• Communicate with property managers and partners.
• Work with multidisciplinary team; attend meetings and trainings.
