Job summary

Qualifications

• BA in human services or equivalent with relevant case management experience.
• Experience coordinating housing programs and working with DMH or similar partners.
• Knowledge of community resources for housing, health, and mental health services.
• Ability to maintain confidential client information and compliance with contracts.

Responsibilities

• Maintain documentation per regulatory and contract requirements.
• Enter data into HMIS and CES systems.
• Prepare progress notes and reports for contracts and audits.
• Collaborate with mental health professionals and DMH partners.
• Conduct intake, assessments, and individual service planning.
• Provide engagement, monitoring, and follow-up for residents and families.
• Support discharge planning and connections to permanent housing.
