Job summary

The Salvation Army Southern California seeks a Case Manager I in Anaheim to deliver holistic, housing-focused case management with a strength-based approach. You will help guests remove barriers, develop housing plans, gather housing documents, and link to on-site and off-site resources to reach sustainable housing.

You will search for housing options, meet landlords, maintain case records in HMIS/WellSky, and participate in weekly CES housing match discussions to update on guests’ progress.

Qualifications

• BSW or equivalent degree preferred.
• Minimum 1 year of case management experience with people in recovery, mentally ill and/or homeless.
• 1 year HMIS experience preferred; familiarity with tenant/landlord relationships.
• Must be in good physical and mental health.

Responsibilities

• Provide holistic, strength-based, harm-reduction case management focused on housing.
• Create individual housing plans and assist with documentation for housing applications.
• Meet with guests four times per month and maintain case notes in databases.
• Coordinate weekly life-skills groups related to housing needs.
• Build rapport with landlords and coordinate housing options and moves.
