Job summary

Making Opportunity Count is seeking a Shelter Case Manager in West Roxbury, MA to provide comprehensive case management for families in shelter. The role uses motivational interviewing and strength-based planning to create individualized rehousing plans and support services to promote safety, stability, and affordable housing.

The position involves collaboration with the Stabilization Program, documentation in ETO, and adherence to program rules and reports.

Qualifications

• Associate's degree in human services or related field.
• 1 year of experience providing case management services focused on pre-tenancy and tenancy-sustaining services.
• Empathetic to the needs of a diverse socio-economic population.
• Strong interpersonal and communication skills.
• Knowledge of community resources, state and federal benefits and services.
• Ability to manage multiple cases and priorities.

Responsibilities

• Establish monthly rehousing meetings with families.
• Provide written notices of meetings per program rules.
• Review barriers to housing and assist with essential documents.
• Create and monitor housing budgets and affordability.
• Document case management efforts in participant files in a timely manner.
• Refer participants to needed support services and resources.
• Report on meetings, housing placements, and program requirements.
