Qualifications

• Experience in housing navigation, care coordination, or case management within community-based programs (3–5 years preferred).
• Strong communication, empathy, and cultural competence are essential.
• Proficiency with case management systems, EHRs, and real-time documentation is required.

Responsibilities

• Provide Housing Stability and Care Coordination services to housing-insecure individuals.
• Coordinate Housing Transition Navigation Services and Housing Deposits with partners.
• Engage in field-based outreach and document interactions in real-time per guidelines.
• Maintain a 60–70 member caseload with proactive outreach and care plan development.
