Pacific Health Group is seeking a field-based Housing Stability Specialist to support Medi-Cal members with housing-related services in the community across counties in California. This role combines care coordination, housing deposits, and navigation to secure permanent housing.

You will spend 30–50% in the field, conduct intakes, develop care plans, and partner with housing authorities, shelters, landlords, and providers, while maintaining real-time documentation and compliance.

Responsabilités

• Spend 30–50% of time in the field delivering face-to-face case management across shelters, residences, and community settings.
• Develop and manage housing care plans and coordinate housing services with providers.
• Conduct intakes, housing assessments, and chart notes in real time per state guidelines.
• Collaborate with housing authorities, landlords, shelters, and healthcare partners to secure stable housing.
• Maintain high-volume outreach and ensure timely enrollment and documentation.
