Job summary

Pacific Health Group is seeking a field‑based Housing Stability Specialist in California. You will support housing placement, navigate housing resources, and coordinate care and services for high‑needs members in community settings.

Ideal candidates have 3–5 years in social services or healthcare, strong communication, and proficiency with EHR and case management tools. Local travel and a CA driver’s license are required.

Qualifications

• 3–5 years in case management, social services, or healthcare.
• Experience with Medi-Cal, CalAIM, and Community Supports programs preferred.
• Excellent communication, empathy, and cultural competence.
• Proficient with case management tech and real-time documentation.
• Willingness to travel locally within the county to conduct in-person visits.

Responsibilities

• Support housing stability through navigation and tenancy services.
• Coordinate housing deposits, landlord engagement, and service connections.
• Perform in-person assessments and ongoing follow-ups in the field.
• Document interactions accurately and in real time per guidelines.
• Collaborate with healthcare providers, housing authorities, and partner orgs.
• Travel up to 60–70% within the county for field visits.
