Job summary

Mercy House in Oxnard, CA is seeking a Housing Solutions Case Manager to support clients in obtaining and sustaining permanent housing under the Housing First model. You will provide comprehensive case management, housing advocacy, progress tracking, and support to help clients achieve housing stability.

Under the direction of the Housing Solutions Manager, you will coordinate referrals, manage a caseload, document using GIRP, and collaborate with partners to address barriers and connect clients

Qualifications

• Bachelor’s degree in Social Work or related field (or four years experience in social work).
• Experience working with individuals experiencing homelessness preferred.
• Proficiency in Microsoft Office and basic computer skills.

Responsibilities

• Maintain an assigned caseload, perform outreach, intake, and housing stabilization planning.
• Provide comprehensive case management, housing advocacy, and client progress tracking.
• Coordinate home visits and co-locate at Oxnard College as scheduled, with coordinations with partners.
