Job summary

The Housing Solutions Case Manager at Mercy House provides ongoing needs assessments, emergency housing plans, referrals, and advocacy for residents in the Permanent Supportive Housing Program. The role includes life skills workshops, community building events, and maintaining thorough records to support housing stability and self-reliance.

The position requires trauma-informed, client-focused practice, strong collaboration with landlords and management, and participation in an on-call schedule

Qualifications

• Bachelor’s degree in social work or related field preferred.
• Experience with mental health, chemical health, and co-occurring disorders desired.
• Ability to use computer software to complete duties efficiently.

Responsibilities

• Maintain a caseload of 20–22 individuals and meet residents regularly.
• Provide trauma-informed, client-centered housing support and advocacy.
• Coordinate life skills, job readiness, and community activities.
