Job summary

Mercy House is seeking a Housing Solutions Case Manager to participate in all stages of client service, from needs assessments to individualized housing plans and advocacy for residents in the Permanent Supportive Housing Program.

The role emphasizes trauma-informed, client-centered care, eviction prevention, and helping residents build self-reliance, with a minimum caseload and regular contacts. You'll coordinate life skills workshops and collaborate with landlords and community resources.

Qualifications

• Bachelor’s degree in Social Work or related field required.
• Experience in mental health, chemical health, or co-occurring disorders preferred.
• Valid CA driver’s license and reliable vehicle required.
• Excellent verbal and written communication skills.

Responsibilities

• Maintain a minimum caseload of 17-30 individuals and monitor housing plans.
• Provide client-centered services using harm reduction and trauma-informed approaches.
• Assist with eviction prevention and housing stability strategies.
• Organize life skills workshops and community-building activities.
• Collaborate with landlords, caretakers, and community resources for services.
• Complete accurate documentation and data collection for reporting.
