Job summary

Mercy House in Phoenix is seeking a Housing Solutions Case Manager to provide support and advocacy for individuals experiencing homelessness. The role involves maintaining a caseload, creating effective housing support plans, and conducting regular client meetings.

The ideal candidate will have a Bachelor’s degree in Social Work or a related field, along with experience in crisis intervention and trauma-informed care. This position offers the opportunity to work directly with diverse populations and make a significant impact in the community.

Qualifications

• Experience with recovery, mental health, and co-occurring disorders.
• Minimum 2 years of direct experience with long-term homelessness preferred.
• Valid AZ driver's license and reliable vehicle.

Responsibilities

• Maintain caseload and develop housing support plans.
• Conduct home visits and community meetings.
• Assist clients in accessing housing and community resources.
• Keep accurate documentation for all client interactions.
