The role involves ongoing needs assessments, individualized housing plans, advocacy, and coordinating access to community resources. Qualifications

• Bachelor’s Degree in Social Work (or related field) preferred, with experience in chemical health, mental health, co-occurring disorders.
• 2+ years of direct life experience with long-term homeless, low-income and diverse populations may apply if BA/BS is not held.

Responsibilities

• Maintain a minimum caseload of 17–30 individuals and develop individualized housing plans based on assessments.
• Provide client-centered services using harm reduction and trauma-informed approaches to help residents achieve housing stability.
• Organize life skills workshops and coordinate job readiness, credit counseling, and community resources.
• Coordinate with landlords, volunteers, and community partners to support resident housing and well-being.
