Job summary

Mercy House is seeking a Housing Solutions Case Manager to support individuals in the Permanent Supportive Housing Program. The role focuses on ongoing needs assessments, emergency planning, advocacy, and life skills development, with a trauma-informed, client-centered approach.

Responsibilities include maintaining caseloads, coordinating housing supports, and linking residents to employment, education, and mentoring services while ensuring accurate documentation and compliance.

Qualifications

• Bachelor’s degree in social work (or related field) preferred.
• Experience with mental health, chemical health, and co-occurring disorders preferred.
• Valid California driver’s license and reliable vehicle required.

Responsibilities

• Maintain a minimum caseload of 20–22 individuals and meet at least twice a month with residents to develop emergency plans and individual housing supporting plans.
• Provide client-centered services using harm reduction and trauma-informed care to sustain housing.
• Conduct home visits, inspect units, and assist with household activities as needed.
• Coordinate life skills workshops (job search, time management, health and wellness) and community activities.
• Maintain accurate case notes, plans, assessments, and confidentiality per HIPAA.
