Job summary

Colorado Coalition for the Homeless in Denver seeks a Case Manager I to collaborate with case managers, integrated health team members, and partners to promote long‑term solutions for families and individuals experiencing homelessness. This role develops care plans, coordinates services, monitors progress, and maintains documentation across systems.

The position emphasizes motivational interviewing, trauma‑informed care, and housing support services, including crisis intervention and referrals.

Qualifications

• High School Diploma or GED required.
• Bachelor’s degree in Psychology, Social Work, Human Services, or Criminal Justice preferred.
• Basic Life Support Certification required within 30 days of employment.
• Colorado driver’s license within 30 days of hire.

Responsibilities

• Manages a caseload of clients with high-service utilization and disabilities including substance use and behavioral health needs.
• Develops and implements care plans, coordinates services, and documents progress in the EHR.
• Provide advocacy, crisis intervention, and linkages to housing and mainstream services.
• Engage with community partners and health providers to support client outcomes.
• Transports clients as needed and maintains safe, trauma‑informed practice.

Education
