Job summary

Qualifications

• High School Diploma or GED required.
• Bachelor’s degree in Psychology, Social Work, Human Services preferred.
• Basic Life Support Certification required within 30 days of employment.
• Colorado driver’s license within 30 days of hire (with the exception of SSHC).

Responsibilities

• Manages a caseload of clients with high-service utilization and disabilities.
• Develops and implements care plans, coordinates services, and monitors client progress.
• Provides advocacy and support, facilitates referrals to other services, and maintains case documentation in the EHR.
