Job summary

The Colorado Coalition for the Homeless seeks a Case Manager I to work with clients experiencing homelessness or at risk. You will develop care plans, coordinate services, monitor progress, and advocate for clients across health and housing resources.

Responsibilities include referrals, documentation, crisis response, and collaboration with partners. Local travel within the Denver metro area and a valid Colorado driver’s license are required.

Qualifications

• High School Diploma or GED required.
• Bachelor’s degree preferred (Psychology, Social Work, Human Services, or Criminal Justice).
• Colorado driver’s license within 30 days of hire preferred.

Responsibilities

• Manage a caseload of clients with high-service needs and disabilities.
• Coordinate services and referrals with community partners and health providers.
• Maintain case documentation and data in the EHR system.
• Provide crisis intervention and de‑escalation in urgent situations.
• Travel within the Denver‑Metro area to meet clients as needed.
