Job summary

The Colorado Coalition for the Homeless is seeking a Case Manager I to work with families, children, and individuals experiencing homelessness. You will develop care plans, coordinate services, monitor progress, and advocate for clients, with a focus on housing-first and trauma-informed approaches.

This role requires travel within the Denver metro area, a valid Colorado driver’s license, and credentialing as needed.

Qualifications

• High School Diploma or GED required.
• Bachelor’s degree in Psychology, Social Work, Human Services, Criminal Justice preferred.
• Must have and maintain Colorado driver's license within 30 days of hire.

Responsibilities

• Manages a caseload of clients with high-service utilization and disabilities including substance use and significant health care needs.
• Maintain positive relationships with community partners and health providers.
• Work under supervision within a team and support problem-solving efforts.
• Support clients with wrap-around care management to ensure engagement and crisis support.
• Facilitate referrals to external services and coordinate care.
