Job summary

The Colorado Coalition for the Homeless is seeking a Case Manager I in Denver to coordinate services, advocate for clients, and support a wrap-around care plan grounded in motivational interviewing and trauma-informed care.

You will work with a team of case managers and health partners to promote housing stability, monitor progress, and document client outcomes while adhering to credentialing requirements and safety protocols.

Qualifications

• High School Diploma or GED required; Bachelor’s degree preferred.
• Experience with homelessness, mental health challenges, or trauma-informed care encouraged.
• Bilingual or multilingual skills preferred.

Responsibilities

• Manage a caseload of clients with diverse needs and goals.
• Coordinate services and resources with partners and health providers.
• Maintain up-to-date case documentation and data in the EHR.
• Provide crisis intervention and support to residents as needed.
• Advocate for clients to access benefits and housing resources.
• Support outreach and engagement with community resources.
