Job summary

Colorado Coalition for the Homeless is seeking a Case Manager I to work with families and individuals experiencing homelessness. The role focuses on outreach, coordinating services, and advocacy to secure housing and health resources in the Denver area.

You will maintain client records, collaborate with health teams, and facilitate referrals while following trauma-informed, housing-first practices. Colorado drivers license required; credentialing process applies.

Qualifications

• Experience in a role serving homeless or at-risk populations preferred.
• Strong communication and advocacy skills are essential.
• Must have Colorado driver’s license within 30 days of hire.

Responsibilities

• Manage a caseload of clients with high service needs.
• Coordinate services and resources across health and community partners.
• Maintain client progress notes and data in the EHR.
• Provide crisis intervention and referral linkage to housing and benefits.
• Develop and implement wrap-around care plans with a strengths-based approach.
• Travel within the Denver metro area to meet clients.
