Job summary

Colorado Coalition for the Homeless in Denver is seeking a Case Manager I to develop and implement care plans, coordinate services, monitor progress, and advocate for residents experiencing homelessness. You will deliver outreach, facilitate referrals to mainstream services and housing, and maintain case documentation.

Travel in the Denver metro area and a valid Colorado driver0s license are required. This role emphasizes motivational interviewing, trauma-informed care, and a wrap-around care

Qualifications

• 15-2 years of experience working with a diverse population who are experiencing mental health challenges.
• Experience working with individuals experiencing homelessness, mental health challenges, or substance use disorders preferred.
• Must have and maintain Colorado driver0s license within 30 days of hire.
• Bilingual or multilingual skills preferred.

Responsibilities

• Manage a caseload of clients with high-service utilization and disabilities.
• Coordinate services and referrals with community partners across the organization.
• Provide advocacy and support, ensuring crisis intervention and de-escalation when needed.
• Maintain timely and accurate documentation of client data and progress in the EHR.
• Travel to meet clients in the Denver/metro area using a CCH vehicle or approved personal vehicle.
