Job summary

The Colorado Coalition for the Homeless is hiring a Case Manager I to work with a collaborative team to develop and implement care plans, coordinate services, and monitor client progress for individuals and families experiencing homelessness or at risk.

You will reference motivational interviewing, housing first, harm reduction and trauma-informed care, conduct crisis interventions, maintain documentation in the EHR, and connect clients to community resources.

Qualifications

• High School Diploma or GED required.
• Bachelor’s degree in Psychology, Social Work, Human Services, or Criminal Justice preferred.
• Experience with diverse populations and mental health challenges preferred.
• Ability to work collaboratively and independently.

Responsibilities

• Manage a caseload of clients with high-service needs and disabilities.
• Coordinate services and referrals with community partners.
• Perform crisis intervention and safety planning as needed.
• Document services and outcomes in the EHR accurately.
• Coordinate transport and accompany residents to appointments.
• Contribute to program development and quality improvement.
