Job summary

St. Vincent de Paul CARES seeks a Navigator for PSH programs in New Port Richey, FL. The role supports intake/discharge, case management, goal setting, and housing navigation using HMIS systems.

You will open/close cases, follow up with clients, and help them reach permanent housing. Requires HS diploma and experience with homeless services; valid FL license preferred. Equal opportunity employer.

Qualifications

• HS diploma or GED required.
• Security training or experience is preferred.
• Experience helping or working with homeless population.

Responsibilities

• Register all new clients using intake folders with demographics and income status.
• Set attainable goals and monitor progress for Case Manager.
• Assist with management of client volunteers.
• Monitor cleanliness in dining area/lounge.
• Assist clients with housing, information, referrals.
• Use HMIS (Clarity) to manage cases.
• Follow up on client progress and report to management.
• Develop Housing Plans with clients aiming for permanent housing.
• Complete paperwork and incident reports as required.
