Job summary

St. Vincent de Paul CARES is seeking a Case Manager IV to provide assessment, planning, and advocacy for clients in PSH/PH programs in New Port Richey, FL.

You will manage a caseload, coordinate with community agencies, and facilitate moves to permanent housing, while ensuring compliance with regulations and program rules. The role requires strong communication, trauma‑informed care, and the ability to work evenings, nights, weekends, and holidays.

Qualifications

• Must be able to speak, write and understand English.
• Basic computer skills required.
• Bachelor's degree and/or 5 years in social services.
• Flexible schedule including evenings, weekends, holidays.

Responsibilities

• Provide crisis intervention for very low‑income, homeless seniors.
• Assess strengths, needs, abilities to set housing goals.
• Ensure compliance with government and contract regulations.
• Assist clients with information, veteran assistance, referrals.
• Coordinate move to permanent housing and link services.
