Job summary

St. Vincent de Paul CARES in Port Charlotte, FL seeks a Peer Mentor for its Rapid Rehousing Program.

You will perform direct outreach, pre-screen applicants, and coordinate with the Case Management team to help participants access community resources and obtain housing. The role requires English proficiency, basic computer skills, and the ability to work with diverse populations; High School diploma required; Level II background screening; benefits include health, vision, life insurance and paid

Qualifications

• Ability to speak, write and understand English.
• Basic computer skills.
• Sensitivity to cultural diversity among clients and staff.
• Ability to multi-task and meet deadlines.
• Proven compliance with background screening requirements.
• Must have reliable transportation and a valid Florida driver’s license.

Responsibilities

• Identify locations frequented by individuals experiencing homelessness.
• Provide outreach and pre-screening for RRH program eligibility.
• Coordinate screening with Case Management team.
• Provide client access to services and resources as needed.
• Monitor and document participant participation and progress.
• Assist in discharge planning and follow-up contacts.
