Job summary

Vincentian CARES in New Port Richey, FL is seeking a Case Manager IV to provide assessment, planning, linkage and advocacy for individuals enrolled in PSH/PH programs.

The role requires strong knowledge of the service delivery system, ability to manage a caseload, adhere to program rules, and coordinate with community agencies to secure housing and supportive services. Evening and weekend flexibility is expected to meet client needs.

Qualifications

• Bachelor's degree or 5+ years experience in social services.
• Veterans preferred.

Responsibilities

• Provide crisis intervention to address the immediate needs of very low-income, homeless seniors.
• Assess clients to develop housing goals based on strengths and preferences.
• Enforce program rules to ensure compliance with regulations.
• Assist clients with information, referrals, and veteran assistance as needed.
• Coordinate move to permanent housing and link with services.
