Job summary

The Salvation Army Southern CA Division is seeking a Case Manager to provide ongoing assessment, planning and coordination to help residents secure permanent housing in Los Angeles. You will evaluate housing, health, financial and psychological needs and develop individualized service plans with progress monitoring.

Qualifications

• Bachelor of Arts in Social Work or related field.
• Experience with low-income and/or homeless populations preferred.
• Must pass criminal background check.
• Bilingual English/Spanish preferred.

Responsibilities

• Provide strength-based case management and service coordination to assist clients in obtaining and maintaining permanent housing.
• Maintain and update client records and HMIS data accuracy.
• Compile periodic reports for TSA and funding agencies.
• Develop and monitor individual service plans and progress.
• Assist clients with access to entitlements and health care services.
• Ensure HIPAA compliance and coordinate with partner programs.
