Job summary

The Salvation Army North and Central Illinois Division is seeking a Case Manager at Evangeline Booth Lodge to support homeless families with housing information, planning, and related services. You will collaborate on comprehensive case plans and provide counseling, referrals, and advocacy to help clients transition to permanent housing.

Qualifications

• Bachelor’s degree in social work, human services, or related discipline.
• One year of case management experience preferred; at least five years working with the homeless as a case manager.
• High School Diploma required.

Responsibilities

• Case management to prevent homelessness; develop comprehensive case plans.
• Conduct weekly case plan meetings with clients.
• Provide outreach, counseling, advocacy, and referrals as needed.
• Maintain written and computerized records of service delivery and HMIS reporting.
