Job summary

The Salvation Army USA Central Territory seeks a Social Services Case Manager to support families at the Lodge and in the community, delivering housing information, planning, and supportive services in a team setting.

You will conduct weekly case plan meetings, assist TANF enrollment, coordinate with CPS and community resources, and maintain compliant documentation to aid clients toward stable housing.

Qualifications

• Bachelor’s degree in social work, human services, or related discipline.
• One year of case management experience preferred; at least five years serving homeless populations.
• Knowledge of housing resources and public assistance programs beneficial.

Responsibilities

• Case Management: assess needs and develop individualized plans.
• Provide housing information, referrals, and advocacy.
• Conduct weekly case plan meetings to monitor progress.
• Coordinate with agencies (CPS, schools) to arrange services.
• Prepare and maintain case records and HMIS reporting.
• Assist with TANF enrollment and financial assistance linkage.
• Ensure compliance with program policies and ethical standards.
• Monitor client progress and support transition to permanent housing.
