Job summary

The Salvation Army North & Central Illinois Division is seeking a Case Manager at the Evangeline Booth Lodge to provide critical social services to homeless families. You will develop and monitor case plans, assess needs, and coordinate housing, medical, and educational services in collaboration with partners and staff.

Responsibilities include outreach, advocacy, documentation, and ensuring compliance with program policies while supporting clients toward permanent housing and stability.

Qualifications

• Bachelor’s degree in social work, human services, or related discipline.
• At least five years of experience working with the homeless as a case manager.
• High School Diploma required.
• Experience with TANF and HMIS preferred.

Responsibilities

• Provide comprehensive case management to prevent homelessness and move clients to permanent housing.
• Develop and monitor individualized case plans including budgeting, housing, medical, and education needs.
• Maintain confidential records and ensure compliance with policies.
• Collaborate with CPS and other agencies to coordinate services.
• Conduct assessments, outreach, advocacy, and referrals to support clients.
