Colonial Group is seeking a Clinical Coordinator, Permanent Housing, to supervise a team of Case Managers and a Program Coordinator, applying Housing First and Harm Reduction strategies to ensure high-quality client care. The role oversees intake, assessments, and service plan development while ensuring timely documentation and regulatory compliance.

Qualifications

• NYS LMSW required; 3+ years with related populations.
• Prior supervisory/administrative experience preferred.
• Clinical understanding of Housing First, Harm Reduction, motivational interviewing, trauma-informed care.
• Experience with crisis intervention, risk assessments and incident management.
• Ability to work with diverse internal and external stakeholders.
• Proficiency with Word, Excel, Outlook and Teams; familiarity with Access databases.
• Valid NY or NJ driver’s license preferred.
• Bilingual preferred.

Responsibilities

• Supervise a team of Case Managers and Program Coordinator.
• Provide regular, structured supervision meetings for direct reports.
• Promote Housing First, Harm Reduction, CPI, Motivational Interviewing, and trauma-informed care models.
• Develop and implement individual service plans; ensure comprehensive, timely documentation.
• Interview and assess potential clients.
• Provide clinical direction and leadership; troubleshoot and strategize on engagement and challenging behaviors.
• Oversee development and implementation of service plans; ensure documentation of client contact and progress.
• Coordinate delivery of care with multiple providers and outreach teams.
• Support and guide front line staff in managing difficult situations and deescalating conflict.
