Colonial Group is seeking a Clinical Coordinator for Transitional Housing. You will supervise Case Managers and ensure adherence to Harm Reduction and Housing First strategies, directing clinical interventions to provide high-quality client care.

Strong documentation and regulatory compliance are essential. The role requires LMSW licensure, at least 3 years in related populations, and the ability to lead a team in a fast-paced environment.

Qualifications

• NYS Licensed Masters Social Worker (LMSW) is required.
• Minimum 3 years of experience with related populations; supervisory and administrative experience preferred.
• Strong clinical understanding of Housing First, Harm Reduction, CPI, Motivational Interviewing, and trauma-informed care.
• Experience with crisis intervention, risk assessments, and incident management.
• Ability to work with diverse stakeholders and manage multiple priorities in a fast-paced environment.
• Proficiency with Word, Excel, Outlook, and Microsoft Teams; familiarity with Access databases.

Responsibilities

• Supervise a team of Case Managers.
• Provide regular, structured supervision meetings for direct reports.
• Promote strategies for case management aligned with Housing First and Harm Reduction models.
• Develop and implement individual service plans; document client contact and progress.
• Interview and assess potential clients.
• Provide clinical direction and leadership; troubleshoot and strategize around engagement and challenging behaviors.
• Promote housing placement readiness and success.
• Oversee development and implementation of service plans; document progress.
• Assess and evaluate client functioning.
• Supervise and conduct case conferences.
• Coordinate care with multiple service providers and outreach teams.
• Collaborate with outreach teams to prevent homelessness recurrence.
• Support front-line staff in de-escalation and conflict management.
• Perform other duties as assigned.
